Diabetes

Diabetes is the most common chronic disease at GFHS, affecting 11% of its patients. The Gaston Diabetes Center, operational since 1999, offers a comprehensive diabetes management program providing diabetes education for any Gaston County resident in need. The Gaston Diabetes Center offers organized classes in English and Spanish as well as one-on-one counseling for diabetes and nutrition. The Gaston Diabetes Center was Gaston County's first American Diabetes Association (ADA) recognized Diabetes Self-Management Program.

In addition, GFHS conducts off-site diabetes education programs in community churches, family practices, and worksites and coordinates with local ophthalmologists and podiatrists who volunteer their time, to provide specialist services to uninsured patients of Gaston Family Health Services.

In 2004, Gaston Family Health Services was invited to participate in a National Diabetes Health Disparities Collaborative. Participation in the collaborative provides GFHS with a systematic approach to healthcare quality improvement activities.
